Form Review Overview

The results are in!

More than 100 individuals from SEEDS districts registered to participate in this year’s Form Review process! Thank you to everyone who signed up and shared valuable feedback. Below is a summary of the input collected and the resulting decisions.

 

A6-SLD - Specific Learning Disability Criteria Form

  • Identified Issue:  Frequent form changes can make it difficult for the SEEDS team to efficiently review forms for accuracy.

  • Resolution:
    • 37% - No Changes ✅
    • 29% Split into 2 forms
    • 24% Split into 4 to match DPI
    • 10% Other

A6-SL - Speech Language Disability Criteria Form

  • Identified Issue: Due to the complexity of the Speech Language criteria, ongoing feedback continues to be received regarding form structure and changes.

  • Resolution:
    • 53% - No changes ✅
    • 36% - Match DPI's Form
    • 11% - Other

I-1 - Invite/ I-2 Cover Page

  • Identified Issue(s): The “Purposes of the Meeting” section can be confusing and does not align with DPI formatting.

  • Resolution:
    • 97% - Match DPI's Form ✅
    • 3% - Other

A-3 Consent to Eval Received Date

  • Identified Issue: The A-3 Received Date is required for TIE records and is currently pulled directly from the form. Many districts do not return to edit the form after it has been signed to add the received date.

  • Resolution:
    • 62% - Pull date from the Student Detail View ✅
    • 21% - Leave as is
    • 16% - Other

A-3 Consent to Test / A-4 No Testing Needed 

  • Identified Issue: Amending these forms can be confusing due to validations and timeline requirements. Users also expressed confusion regarding which sections should be completed at different stages of the process.

  • Resolution:
    • Further investigation needed. ⚠️
    • The feedback results for these forms were not conclusive; however, it was clear that users would like to see updates and shared many thoughtful ideas for improvement. Due to the complexity and scope of the potential changes, this topic will be brought back to the team for additional review next school year.

Overall, this new format for reviewing forms was extremely successful. Participants helped identify needed changes and provided valuable feedback to guide future improvements.

WISEdata Updates

SEPA End Date Task - NEW

Schedule your end date!

Districts can now schedule when SEPA records will end. 

  1. Go to Toolbox- WISEdata Settings. 

  2. Add the last day of school to “End date for 25-26 school year”

  3. Click Save End Date

  4. Click Schedule End

  5. Add the date that you want to stop sending SEPA records for the school year. 

Be sure the end date matches the last day of school in the SIS.

It's time to request your 26-27 Ed-Fi Credentials from WISE! Directions can be found here. This will also be discussed at the Year Close Out Webinar on May 27 and June 4.
